Once a defect is marked fixed it is QA's job to verify the fix. If their
tests fail then they mark it as a failed which sends it back to the
opened state.

I think QA did the test wrong. They have some new folks testing the
upgrade and forget that the broken upgrade is installing the fix and, as
it looks in this case, repeat the broken process.

I think the notes section or the regression focus section (on the
resolution page) should have some instructions telling QA not to do the
system copy all -I test until after the new verify_install.sh is
installed ( two upgrades are needed.)=20

Now see, if cmd_upgrade would run the verify_install.sh from the
distribution we wouldn't be having this conversation... ;-)

I'm just going by what I read in the notes.  I did mark this one as
fixed after I checked in that change.  I just didn't recognise it.
What's the story?  Why was it un-marked as being fixed?
