I just completed my analysis and the short answer is just what I
originally speculated: the vendor has switched to a new supplier and it
isn't an approved/known/qualified (by Software Dev.) vendor.  It's a
bit tricky, since the running kernel can deal with it, so the card can
be inserted into a running system and flash_install will build it, but
it won't boot.

I'll make a note in the bug.
